• Print this form and post to the Society Administrator,
  c/- PO Box 70, Kaikoura New Zealand 7340
  to arrive no later than 5 pm on 13 November 2008.
• Print this form and fax to Society Administrator, (+64 3 319 7211)
  to arrive no later than 5 pm on 13 November 2008.
• Hand a signed acceptance form to any Executive member before the meeting.
• Signify acceptance in person at the meeting.
